Photography and Video Release
I authorize Bee Fit CrossFit & BootCamp, LLC DBA Angry Bee CrossFit & BootCamp ("Angry Bee") to take photographs, videos, and recordings of me and my voice, conversations, grunts, groans, and other sounds made during and in connection with Angry Bee activities. I acknowledge that Angry Bee shall own exclusively all copyright and other rights in and to such photographs, videos, and recordings and may use them in any manner without compensation to me. I authorize Angry Bee to use my name, voice, likeness, and any biographical facts disclosed to Angry Bee in advertising and promoting Angry Bee specifically and/or CrossFit activities generally without compensation. I Agree to the Photography and Video Release.

Consent/Assumption of Risk
I, the undersigned or the minor I am signing for, understand and agree that by participating in Angry Bee activities, face significant risks of injury. These risks may include, but are not limited to: (1) loss or damage to personal property; (2) injury or death due to the negligence of myself or other co-participants; (3) heat exhaustion; (4) dehydration; and (or) (5) exposure to various on- and off-site premises and/or equipment and all the inherent risks therein, including slips, falls, collisions, and falling objects. (6) Any injury during a WOD or other activity in the gym.

I understand and assume all risks EVEN IF RESULTING FROM NEGLIGENCE, AND EXPRESSLY AGREE TO WAIVE ALL NEGLIGENCE CLAIMS. I further represent that I have made Angry Bee aware of any and all current or past physical or mental disabilities, injuries, and/or problems which might limit my participation and I will advise Angry Bee and its coaches at any point when I question my ability to participate in any activities.

I EXPRESSLY AGREE AND INTEND THAT MY PARTICIPATION IN ALL CF LODO, LLC ACTIVITIES AND USE OF ALL EQUIPMENT AND FACILITIES SHALL BE UNDERTAKEN AT MY OWN RISK. 

Release
In consideration of the above-mentioned risks, I hereby acknowledge that I freely and voluntarily wish to participate in the fitness, instruction, and activities of Angry Bee and use its facilities, equipment, premises, and coaches. In doing so, I hereby release Angry Bee their principals, heirs ,agents, employees, and volunteers from any liability, claims, demands, actions or rights of action which may arise out of or in connection with my participation in Angry Bee activities whether from acts of active or passive negligence on my part or the part of anyone else. This agreement shall be binding upon myself, my spouse, children, parents, heirs, assigns, legal representatives, and any other person claiming under, by, or through me.


Indemnification
I accept financial responsibility for any injury caused to myself or to any other participant due to my own negligence, or during the course of the workout w/o negligence: just active participation. Should the above-mentioned parties, or anyone acting on their behalf, be required to incur attorneys fees and costs to enforce this agreement, I agree to reimburse them for such fees and costs. I further agree to indemnify and hold harmless Angry Bee their principals, heirs agents, employees, and volunteers from liability for any such injuries, deaths, and damage to property that may result from my negligent or intentional act or omission while participating in Angry Bee activities at the main building or elsewhere, including but not limited to parks, recreational areas, areas adjacent to main building, and/or any area selected for training by Angry Bee. The terms of this Waiver and Release of Liability are to be governed by and construed under the laws of the State of Texas.

In the event any term or provision of this Waiver and Release of Liability is found to be unenforceable or void, in whole or part, the term or provision concerned shall be construed as valid and enforceable to the maximum extent permitted by law, and the balance shall remain in full force and effect. I agree that exclusive venue for any dispute between Angry Bee and me involving this Waiver and Release of Liability shall be in Collin County, Texas.
I have read the foregoing Waiver and Release of Liability and I understand that by signing it obligates me to indemnify the parties named for any liability for injury or death of any person and(or) damage to property caused by my negligent or intentional act or omission. I understand that by signing this form, I am waiving valuable legal rights.  advises Angry Bee advises you to discuss w/attorney prior to signature if you do not understand this agreement in its entirety. I am not under duress or coercion and of sound mind and body .
